In compliance with the Ohio Youth Soccer Association – North
(“OYSAN”) LeagueOne Electronic Registration system, the following records are
to be obtained and verified by the Community/Club listed above:

Registration Form containing current and accurate information, signed by
parent/guardian

Player proof of age supplied by a government certified document, reviewed and
verified by the Community/Club registrar

Medical
Release Form, signed by parent/guardian
Ø
NOGSL
Code of Conduct form signed by each player, coach and manager rostered to a
team and co-signed by parent/guardian if member is a minor

OYSAN
Coach/Volunteer Registration Form
Ø
Printed
confirmation of Risk Management registration for each adult rostered to a team
The Community/Club listed above agrees to abide by the rules
of the Northern Ohio Girls Soccer League (“NOGSL”) and the Ohio Youth Soccer
Association – North (“OYSAN”) and recognizes the following restrictions:
Ø
No
player may be rostered to a team that is more than two (2) years older than her
appropriate age group
Ø
No
player may be rostered to a team that is younger than her appropriate age group
Ø
A
maximum of three (3) out of community players may be rostered to a team unless
a written request for exemption is approved and signed by the NOGSL board of
directors
As authorized officers/agents of the youth soccer
club/organization listed above, we attest that this Community/Club has secured
and verified the required documentation and signatures from our member players,
their parents/guardians, coaches and managers who are rostered to teams registered
to participate in Northern Ohio Girls Soccer League (“NOGSL”) activities for
this seasonal year. The Community/Club
will enforce the rules and restrictions set by NOGSL and OYSAN. In addition, we hereby release, discharge
and/or otherwise indemnify Northern Ohio Girls Soccer League (“NOGSL”), Ohio
Youth Soccer Association North (“OYSAN”), U.S. Youth Soccer (“USYSA”) and the
United States Soccer Federation (“USSF”), their affiliated directors, officers,
employees and agents, including the owners of fields and facilities utilized by
the program against claims by or on behalf of individuals or groups as a result
of our failure to enforce the rules and restrictions set by NOGSL and OYSAN or maintain
true and accurate records for participation in NOGSL-sponsored activities
